Talbert House Receives Certified Community Behavioral Health Clinic (CCBHC) Expansion Grant

Talbert House is excited to announce the 3.5 million dollar awarding of the Certified Community Behavioral Health Clinic (CCBHC) Expansion Grant from the Substance Abuse and Mental Health Services Administration (SAMHSA) to increase access to care and enhance the quality of mental health and substance use disorder treatment in Hamilton County. The CCBHC grant will offer integrated behavioral health and primary care services at three locations: Walnut Hills, Western Hills and The ADAS Center.

Talbert House will implement evidence-based integration practices and partner with Health Care Access Now (HCAN) for care coordination. The project will focus on our most vulnerable populations and communities serving youth and adults with serious mental illnesses or substance use disorders along with any co-occurring disorders.

Data shows 68% of adults with mental health conditions also have chronic medical conditions such as heart disease and diabetes. Integrated care is the most effective treatment to improve health, lower the costs of uncoordinated care, and provide necessary medical accessibility within our community. Treatment plans will include physical, behavioral, wellness, and social goals along with the assistance of individual/group counseling, medication-assisted treatment, medication management, supportive community services, case management and peer recovery support to promote patient sustainability and independence.

Several organizations within the community will collaborate with Talbert House to offer integrated services including the University of Cincinnati’s 24-hour Mobile Crisis Response Team, Greater Cincinnati Behavioral Health ACT, New Path Child & Family Solutions and Central Community Health Board.
